We are a non-profit community resource and support system providing holistic life and death care.

Our ultimate goal is to offer our community a network of support services to prepare and care for its residents at end-of-life under a social model of hospice.
What is a Social Model Hospice?
A social model hospice (SMH) home provides care to residents at end of life who have physical, emotional and financial burdens not addressed in the current medical model hospice (funded by Medicare and commercial insurance) we are all accustomed to. The SMH involves a cultural shift in the delivery and quality of end-of-life care and is accessed when the care necessary at home exceeds the individuals support network of family and friends and is not able to be addressed through a medical model hospice.
The SMH provides autonomy to dying patients by providing care in your home or a home-like setting free of charge or on a sliding scale through both paid caregivers and volunteers. SMH allows family members to be in their familial role during this finite and meaningful time.
What we know is people, if given the choice, want to die at home. But often the needs associated with achieving this goal go beyond what an individual has for resources, support systems and advocacy.
The Sage Heart and its founders, Lisa Cassidy and Carmen Peace have partnered with Omega Home Network a non-profit national membership organization that promotes the development of community homes for dying people.

How Does The Sage Heart Fit In?
We see our organization, The Sage Heart and its tenets, as the piece that is missing in the way we care for our loved ones at end of life in our modern society. We want to generate community support for a program that cares for the community and all its members no matter their circumstances.
With a looming caregiver crisis, caring for the needs of a loved one can be overwhelming. Having a resource to provide support will be vital.
Our goal as a non-profit is to provide services needed for end-of-life planning and one on one care at the bedside when you are in your final days either in your home or at ours.

The Sage Heart offers practical advice for living life while facing death. The reality is none of us know if we have tomorrow. So, with that in mind, we should plan. All of life’s major events require planning and death is no exception. Taking the time to open up about what is important can be such a gift. A gift in the present to enhance our connections to our loved ones and a gift for the future when those we choose to speak for us and care for us never have to wonder if they are doing the right thing. Because we’ve already told them.
A Different Approach

The Sage Heart can help you approach these discussions in many ways. Hosting group ‘Death Discussions’, providing individual consultation for preparing paperwork and important documentation such as advanced care planning, talking with family about illness, hospice & palliative care, contemplative writing for eulogy or obituary, thinking about choices for your body and your funeral, funeral celebrants, navigating grief and many many other details.
Meaningful Conversations
24/7 Support
Having difficult but meaningful conversations, focusing on what is important in your life and how that can be a through note to focus on what is important to you at end-of-life. These are the discussions we find hard to have but ones we can reap so much reward from. The Sage Heart provides support, advocacy, guidance and expertise in the steps you need to take to live life fully while preparing for death. And there’s no time like the present.
Having conversations about death, its meaning and its mystery can be scary. But they don’t have to be. Taking the time to have the hard talks now, gives you more time to just be present when illness does present itself and time is short. Allow yourself the freedom to live life to the fullest at any stage.
Help - How and When You Need It
How Can The Sage Heart Help You?
Lisa and Carmen are available to meet with you individually, in family/friend groups, in the community (library’s, church’s, coffee shops, place of business, etc.) and discuss facets of life & death care that are important to you. The Sage Heart is also pragmatic and knows that needs can go beyond the conversations and the paperwork. Sometimes you need a ride to an appointment or a hand in the kitchen and not everyone has a willing and able caregiver.

Through generous donations and community support we can achieve this innovative, and at the same time ancient, approach to caring for one another. This model is forged by the community, for the community.

We see all care as palliative care through the continuum of the human experience. Empathy drives human connection and alchemy is the art of transformation, inner liberation and change. Changing how we think, act and approach the way we care for each other in life and in death can be a shared community experience, which will enrich all of our lives. The Sage Heart serves our community with experience and guidance for each step along the way for you and those you care for.
